Saving the tooth comes first

Your own tooth is almost always better than the best replacement, so an extraction is advised only when a tooth cannot be kept. A badly decayed tooth can often be saved with root canal treatment and a crown, and a broken one can sometimes be rebuilt.

Wisdom teeth: when they need to come out, and when they do not

Not every wisdom tooth needs removing. One that has come through straight, can be cleaned and causes no trouble can stay.

Removal is advised when a wisdom tooth is impacted against the tooth in front, keeps causing infections in the gum flap over it, is decaying because it cannot be cleaned, or is damaging the neighbouring tooth. An impacted wisdom tooth needs a small surgical procedure. Complex cases are handled by a panel of specialists working as a team, under one treatment plan.

Planning for the gap before the tooth comes out

A gap left unfilled lets the neighbouring teeth tilt into it and the opposite tooth drift towards it, which can change your bite over the years, and the bone where the tooth was slowly shrinks.

The first 24 hours

Bite firmly on the gauze for 30 to 45 minutes. For the rest of the day do not rinse hard, spit, use a straw or smoke, because each of these can dislodge the blood clot that protects the socket.

Eat soft, cool food on the other side, use a cold compress on the cheek for swelling, and take the painkillers the dentist recommends. Call the clinic if bleeding has not slowed after a few hours, or if the pain gets worse rather than better after the third day — that can be a dry socket, which is treatable.
